Drainage Variation Request Form

This form is to be used when:

  1. Further earthquake damage is discovered during the repair process.
  2. Additional work is required to effect the repair strategy.
  3. In the event that a variation is required, due to either of the above scenarios, to the degree possible all work to the affected area should be stopped in order to seek approval from EQC.
  4. A registered drainlayer must complete the Variation Report, Drainage Variation Costing Sheet, and Reinstatement Variation Costing Sheet (only required if applicable).
  5. The drainlayer must include the customer’s email address where noted below so the customer has an opportunity to view the report.

Once submitted, EQC will process the request as quickly as possible so the repair can continue. If the repair works continue without approval, the work may not be covered.

For any drainage repair variation questions, contact your settlement specialist/claim manager by either:

Contacting the staff member assigned to the claim directly
Calling 0800 DAMAGE (326 243) between 8:00am and 4:30pm, Monday to Friday
