Information Request Form

We will respond in accordance with the Official Information Act (OIA), Privacy Act, Earthquake Commission Act or all (whichever is appropriate to your request) as soon as practicable and in any event no later than 20 working days of receiving your request.

Please be aware that the Ombudsman Holiday period began on 29 November 2021.

We will endeavour to provide a response to you within 20 working days from receipt of your request, the latest date of the response being Monday 14 February 2022.

Definition of an authorised requester

Authorised requesters include: the insured/claimant, claim contact, current property owner with Deed of Assignment (DOA), or authorised representatives including real estate agents representing the seller, with a listing agreement or requests related to you personally

Unauthorised requesters include: a potential home buyer, current property owner without a DOA, or other non-claim contact parties with demonstrated interest in receiving information.
