Information request form

We respond in accordance with the Official Information Act (OIA), Privacy Act, Earthquake Commission Act or all, whichever is appropriate to your request. We're required under the OIA to give you our decision on your request as soon as possible, and no later than 20 working days after we receive your request.

The Official Information Calculator on the Office of the Ombudsman's website(external link) will help you count the time a request is likely to take.

Are you authorised to receive this information?

Definition of an authorised requester

Authorised requesters include: the insured/claimant, claim contact, current property owner with Deed of Assignment (DOA), or authorised representatives including real estate agents representing the seller, with a listing agreement or requests related to you personally

Unauthorised requesters include: a potential home buyer, current property owner without a DOA, or other non-claim contact parties with demonstrated interest in receiving information.
